Daniel is young man in his 20s who first presented at pathways in May. At the time of referral he was staying a tent having became homeless after his drug use caused a relationship breakdown with his parents. Daniel had historically used a variety of drugs but at time of referral was 2 years clean from these drugs.
 
Daniel is diagnosed with a range of mental health conditions and struggled to manage his personal life and his emotions. A referral was made to our specialist mental health nurse who spoke with Jack on a number of occasions.
 
Pathways made several referrals to supported accommodation providers. One of these providers was Derventio housing who assessed Daniel and accepted him. He was placed onto a waiting list until a suitable property became available and the 1st July Daniel moved into the property.
 
Daniel is now doing well and is settled in his new home.
